Complex care
Specialist, person-centred community support for adults with complex physical health, neurological, long-term and high-dependency needs.
Support is delivered at home and in the community by trained, competency-assessed teams, working alongside families, commissioners and the relevant healthcare professionals.
Who it is for
Complex Care supports adults aged 18 and over whose needs require skilled, planned and consistent support.
What the package includes
Every package is built around the person, then supported by the systems that keep it safe and consistent.
Plans are built around the person’s needs, routines, communication and the outcomes that matter to them.
Colleagues complete role-specific training and competency assessment before working unsupervised.
Medicines are supported in line with the care plan and prescriber’s instructions, with recording, audit and review.
Where appropriate, proactive and least restrictive strategies support quality of life and meaningful participation.
We work alongside families, commissioners, therapists and the relevant NHS and healthcare professionals.
Where a delegated healthcare task forms part of an agreed care plan, Registered Nurse leadership and clinical oversight apply.
Governance
Assurance is built into how packages are set up, recorded and reviewed.
Conditions
These are examples rather than a complete list. Support is always planned around the individual, not the diagnosis.
Clear, accountable support
Complex packages work when everyone knows what is happening, who is responsible and what changes.
Appropriately trained and competency-assessed care staff work within clearly defined protocols, with Registered Nurse leadership and clinical oversight where applicable, and in partnership with the relevant NHS or healthcare professionals.
Tell us about the person and their circumstances. We will explain the assessment process and give an honest response about capacity, mobilisation and next steps.
