Professionals & referrals
Ultimate Complex Care works with Local Authorities, NHS organisations, ICB teams, social workers, case managers and other professionals to arrange personalised community care for adults aged 18 and over. We provide open information about capacity, assessment requirements and realistic mobilisation timescales.
Our services span Home Care and companionship, Supported Living, Live-in Care, Complex Care, specialist disability support and delegated healthcare support where appropriate.
One provider, connected support
Referrals range from everyday Home Care to specialist Supported Living and Complex Care. Each package is planned with the individual and the professionals involved, supported by clear documentation, risk management, competency assessment and outcome-focused reviews.
Who we work with
We accept referrals from across health, social care, housing and the third sector.
What we can take
Packages can combine more than one of these, and can move between them without changing provider.
A clear referral route
Four stages, each with a clear owner and a clear handover.
Governance & accountability
The evidence behind a package matters as much as the package itself.
Whether you are exploring an immediate need or planning future support, we will provide an honest response about suitability, capacity, assessment and next steps.
Referral questions
Use the referral form on this website, call 01934 416585 or email enquiries@ultimate-complex-care.co.uk. We will confirm what we need and arrange the initial conversation.
The person’s circumstances and required outcomes, location, funding route, urgency, current professional involvement, and any known risks, communication needs or equipment requirements.
It depends on the assessment, the commissioning arrangement, the competencies required and our capacity at the time. We give an honest answer on timescales when we take the referral rather than a fixed start date.
Yes. We work with Local Authorities, ICBs, continuing healthcare teams and private funders, including packages funded jointly. Responsibilities are agreed in writing before support begins.
Appropriately trained and competency-assessed care staff may support delegated healthcare tasks where these form part of an agreed care plan, within defined protocols and with Registered Nurse leadership or clinical oversight where applicable. UCC works in partnership with relevant NHS and healthcare professionals and does not replace GPs, District Nurses, specialist nurses or other NHS clinicians.
Through agreed review arrangements, incident and safeguarding reporting routes, and direct professional communication. Reporting frequency and format are agreed when the package is set up.
