Referrals and enquiries
Refer an adult aged 18 or over for personalised community care and support. Individuals, families and professionals can use this initial referral form to tell us what support may be needed.
Our services range from Home Care and companionship to Supported Living, Live-in Care, Complex Care, specialist disability support and delegated healthcare support where appropriate. We will provide an honest response about suitability, capacity, assessment requirements and next steps.
Initial referral
This form collects the initial information needed to understand the enquiry. Please do not include detailed medical records, reports or other highly sensitive information. If further information is required, our team will explain how it can be shared appropriately.
Ultimate Complex Care is not an emergency service. If someone is in immediate danger or requires urgent medical assistance, call 999.
After you refer

Privacy and information
We use the initial information to understand the enquiry and arrange an appropriate follow-up. Detailed clinical records, assessments and other sensitive documents should only be shared through an agreed secure route when requested.

Referral questions
Individuals seeking their own support, family members and friends, social workers, commissioners, case managers, healthcare professionals and other authorised representatives can all make an initial referral.

Yes. You do not need a professional to refer on your behalf. Where you are referring someone else, we ask you to confirm you have their permission or another appropriate lawful basis.
Referrals are reviewed during office hours. How quickly we come back to you depends on the nature of the referral and team availability, so we do not promise a fixed response time.
No. Every referral is considered individually against our services, current operating area and capacity. Submitting a referral does not confirm that we can accept or immediately mobilise support.
Not through this form. If further information is required, our team will explain and agree a secure route for sharing assessments, care plans or other sensitive records.
