Personalised support for people living with dementia, Alzheimer’s disease, acquired brain injury, epilepsy and progressive neurological conditions.

Transition to Adult Services

Coordinated support for young adults moving from Children’s Services into Adult Services, promoting continuity, independence, family involvement, life skills and community participation.

Delegated healthcare

Registered Nurse Leadership and Delegated Healthcare

As a CQC-regulated provider registered for Treatment of Disease, Disorder or Injury, Ultimate Complex Care can support the delivery and oversight of delegated healthcare tasks where these form part of an agreed care plan.

Appropriately trained and competency-assessed care staff work within clearly defined protocols, with Registered Nurse leadership and clinical oversight where applicable, and in partnership with the relevant NHS or healthcare professionals.

Ultimate Complex Care does not replace GPs, District Nurses, specialist nurses or other NHS clinicians.

It depends on the level and type of support needed. Home Care suits people who need help at home for part of the day, while Live-in Care, Supported Living and Complex Care suit more continuous or specialist needs. Our team will talk through the options with you.

Do you provide straightforward Home Care and companionship?

Yes. Home Care is an important part of our wider service offer. It covers personal care, medication support, meals, mobility, housekeeping, shopping, companionship, appointments and community access.

Can support increase or reduce as needs change?

Yes. Packages are reviewed as circumstances change, so support can increase or reduce while keeping the same provider, care records and, wherever possible, the same team.

How is delegated healthcare managed safely?

Where a delegated healthcare task forms part of an agreed care plan, trained and competency-assessed colleagues work within clearly defined protocols, with Registered Nurse leadership and clinical oversight where applicable, alongside the relevant NHS or healthcare professionals.

Can you work with families, commissioners and healthcare professionals?

Yes. We work in partnership with families and representatives, social workers, case managers, Local Authorities, NHS organisations and commissioners throughout assessment, planning and review.
