Supported living
Support for adults aged 18 and over with learning disabilities, autism, mental health needs or other support needs, living in their own tenancy or home.
Ultimate Complex Care provides the care and support. Housing is arranged separately, so the person’s tenancy rights, choice and control stay with them.
Who it is for
Supported living works where someone wants their own home and support that flexes around it.
What is included
Care and support planning respects tenancy rights, choice and control at every stage.
Understanding a tenancy, managing bills and paperwork, and knowing where to go when something needs fixing.
Cooking, budgeting, shopping, laundry, travel and the practical skills that make a home work.
Building friendships, interests, routines and a place in the local community.
Where appropriate, proactive and least restrictive support that improves quality of life and meaningful participation.
From visiting hours through to more intensive arrangements where available, adjusted as independence grows.
Working alongside families, commissioners, health professionals and housing providers.
Governance
The systems that keep support consistent when someone is living independently.
Areas of experience
These describe experience, not the people we support.
The person’s own journey
Supported living is about the life someone is building, not the tasks on a rota. Support follows the person’s own journey and changes as their goals change.
What that includes is agreed with the person, and reviewed with them as things move on.
Tell us about the person, the housing arrangement and the support they need. We will explain the options and give an honest response about assessment, capacity and next steps.
